Overview
Social Worker/Therapist III
The Social Worker/Therapist is responsible for providing therapeutic support for transitional life events to patients and their families. Participate in the multidisciplinary team. Develops, implements and reviews treatment plans and reviews. Completes psychosocial assessments. Facilitates inpatient and post-acute consults to meet psychosocial needs. Facilitates individual, family, and group therapy. Provides case management and discharge planning while documenting within required time frames. Ability to effectively assess, plan, and implement therapeutic services in a multidisciplinary team settings. Knowledgeable regarding all populations with background in medical social work.
Social Worker/Therapist IV
The LISW Therapist is responsible for providing therapy services to patients and their families as part of a multidisciplinary team. Practices in both inpatient and outpatient settings. Demonstrates ability to provide diagnostic impression. Develops, implements, reviews treatment plans and reviews. Demonstrates ability to provide and update clinical assessments. Communicates verbally and in writing with physicians and other providers. Completes psychosocial assessments. Facilitates individual, family, and group therapy. Provides case management, discharge planning while documenting within required time frames. Ability to effectively assess, plan, and implement therapeutic services in a multidisciplinary team setting. Knowledgeable regarding all populations with background in medical social work.
Qualifications
Social Worker/Therapist III
- High School diploma or equivalent. Current American Heart – HeartSaver or Basic Life Support (BLS) card.
- Masters Degree in Social Work.
- Certification:
- Licensure: At least one year of experience as LMSW, if working under a provisional license.
- Experience in Medical Social Work in a variety of settings, utilization review, discharge planning, and resource management.
- Demonstrated ability to work effectively with patients, families, physicians, and administrations.
- To work well under pressure; to deal with multiple and sometimes conflicting priorities; and to utilize critical thinking skills.
- The ability to process and willingness to learn new computer skills.
- Demonstrated skills with computers and word processing.
- On-call required.
- Performs related responsibilities as assigned.
Social Worker/Therapist IV
- Master’s Degree in Social Work or Psychology with Independent Licensure.
- Demonstrated ability to work effectively with patients, families, physicians, and administration.
- Demonstrated ability to work well under pressure; to deal with multiple and sometimes conflicting priorities; and to utilize critical thinking skills.
- Proficiency in computer skills related to electronic health record and Word processing.
- Current American Heart – HeartSaver or Basic Life Support (BLS) card.
- Certification:
- Licensure: Greater than three years of experience as LISW or LPCC.
- Performs related responsibilities as assigned.
